What to do?

Well I know this isn't the best topic but I am about 20 weeks pregnant and haveing a hard time going #2 I have took some medicine to help soften it but nothing has worked but it has gotten to the point where I may be able to get it out but as I tryed it hurt extremely bad as if I was going to rip and I'm not sure what to do it's difficult I don't wanna get sick for not going but I can't even go. So what should I do? I have talked to my doctor but she is the one who says to take the medicine and it still didn't work.
