Could be a tiny hole in it or if it was a very old condom they can break down. Or a lot of people play around a little pre condom not realizing precum can get the job done

Are you absolutely sure the condom hasnt failed? The only other way would be via precum

Potentially if the guy played with himself then fingered you? If used properly condoms should prevent pregnancy but nothing is 99% effective :)
