Maternity leave

I was talking to my doctor about it and my company gives 16 weeks. My doctor said she can only clear me for 12 bc 16 isn't the norm unless I have some issues after birth. I'm confused I've never been on a leave but how does it work? Are my 16 weeks not guaranteed?! I wanted the max to spend with my baby!
