Driving licence

I have PTSD and smoked weed up until a year ago and its all on my doctors records when i seeked help i was 100% honest. Im now filling out my form for my learners licence and its asking if i have any mental health problems and if I've used any substances in the past 3 years. I don't want to lie and get fined so i ticked yes on both. My neighbour just told me to get a new form and not tell them anything because they will ban me from having a licence like they did to her for 3 years. What should i do, i really want to get some independence and move on with my new life with my fiance and my horrible past of abuse is still got chains on me😞
