Crib Transition

So my baby is 5 weeks old and is sleeping through the night! Due to her reflux she sleeps in a rock and play. Plus she likes to be held and I guess it makes her feel like she is in my arms. How did anyone transfer your baby from an incline to a flat crib? Did you do something to make them feel like they were being held? Any ideas?
