Tampons and Sex

Ok, so, I'm reading through the posts on here and I am confused. Or maybe curious is the right emotion. How exactly does having sex with a tampon in work? Wouldn't your guy feel it? And wouldn't it be uncomfortable for him? Maybe the men I sleep with are on the larger side, but I feel like they would all know and that would have to be uncomfortable (physically) for them.
