first appointment at 18 weeks

I've waited a while to post about this because of the criticism I'm sure to get. I'm 18 weeks as of today and have yet to see a doctor. It isn't that I haven't been trying to go to the doctor; it's just that we have been having a lot of trouble with insurance and what not. Anyway my first appointment is November 9th. I'm pretty nervous and have no idea what to expect. Any ideas? Advice?
