When you went to apply for Medicaid, what all documents did you need?

If you have insurance through work, can you still qualify for Medicaid? If so, can you choose to use medicaid only and not your insurance ? Or is it best to have one as primary and one as secondary?

I'm debating if I really do need insurance through my workplace, I have previously been approved for Medicaid before I signed up for insurance through my workplace. So I still have my medicaid card valid this year (2016).

I think my insurance is only covering 80% of costs so I will still have to pay 20% out of pocket and that is still a lot coming from how much I get paid.

Any help and suggestions will be oh great help. I don't want to stress about this anymore.
