Lip Tie-Baby is 1 month old

My husband and I took a look at our daughter to see if she has a lip tie and to our discovery it certainly appears that she does! (I had stopped breastfeeding for about 1 week already due to many problems) She is now 1 month old, not sure if it's too late to have it removed or if the insurance even covers it. I don't think my husband's insurance even offered us dental for her it was just medical.
Why was this not brought to our attention at the hospital when she was born by the pediatrician, nurses or lactation consultants?!
Would this explain why I fed her on demand for 2 weeks and never produced enough milk when pumping during the day and sometimes late night? Would this also explain why she would actively eat for 10 minutes than fall asleep but stay suckling at my breast for another 20 minutes and when she popped herself off because I thought she was done and lay her down to sleep, shortly after about 5-15 minutes later she would wake up hungry again? And would this explain why she had trouble gaining her birth weight back even though I was breastfeeding on demand?
