best way to introduce formula and to dry up breast milk?

I'm planning on stopping breastfeeding in the next few weeks and I want to start introducing formula slowly so that my newborn can get used to it and i don't just throw it on her and upset her stomach. I've heard mixing it with breast milk helps but what's the best way to do it? Like do I warm the formula and the breast milk seperatly ? The baby water is room temp and my breast milk is in the fridge... & how should I start weaning my baby from breastfeeding & start drying up my milk supply? I have about 2 weeks before I go back to work.. my baby is a month old.

I make really hot formula and add fridge breast milk or if I have room temp milk I make the formula with warm water.. For my daughter I started doing mostly breast milk and added more formula and less breast milk slowly and now she's on mostly formula with a little milk.. At night she gets straight formula cuz it's easier to make and I'm tired. She's only still getting breast milk cuz I have it so might as well use it. And it's gone really well this way and she barely spits up and isn't fussy. As for drying up the milk I've put myself on a schedule to gradually reduce how much I pump.

You could so it two ways. 
1. Warm the breastmilk and add it to the formula at just right warm temp. 
2. Mix the formula powder with boiling water and then add the breastmilk from the fridge and that should make it the right temp. 
Don't heat them up together as you need to be sure the water is hot enough to mix powder properly. Hope that helps 😊
