bleeding during sex

So recently I have been bleeding during sex, not too much but enough that I can notice it. The last time was three months ago with my partner and there was a red spot on the condom and then today without a condom and I still bled. It's really freaking me out. Could it just be dryness? Ive had sex in really weird positions and never have I ever bled so idk whats up
