For people saying it was somehow related to how she looked or what she did, that's ridiculous and not true. Sometimes that happens to guys if they're not focused, or too focused on trying not to orgasm that they may lose the erection. If that happens again, tell him it's ok, maybe offer oral or hand stimulation if he's willing to try again? Sometimes it's just a fluke and won't happen then next time.
