Appointment today since miscarriage

I finally have my appointment check up since I had a miscarriage and my manager said I need a doctor's release form to return back to work I was planning on going back into work tomorrow but I really don't think I'm ready to go back it's still hard for me and my emotions are still everywhere now my question is for the release form to turn back to work what they put in the date that you are ready to return back to work or will they just put in the date that you had your doctor's appointment cuz I'm really not ready to return back to work tomorrow I really don't think I'd make it through the day
