Birthing Plans?

Looking for free birthing plan template -has anyone found good ones?  Most I come across cost $$$ or aren't very comprehensive.  What have everyone considered putting on theirs??

I didn't use a template. Just listed the things I wanted or didn't want. Keep it short and sweet and to the point.

I made one from mama natural but then made a visual one. The visual one I plan to freaking post everywhere,  doors, monitors, etc. With my first son not a soul read my plan, no one listened, it was frustrating to say the least. I'm beyond done with hospital staff pushing requests to the side! This is my story, my body, my baby! You will treat me with respect and within healthy bounds follow my wishes. 
I'm hoping pictures will get the point across :)

My nurse said I didn't need one. Should I make one anyway? I figured they probably won't read it. They don't even read their own notes to each other in hospitals!
