Questions while at doctor

Alright ladies I am finally going to the Dr after almost 4 years(I know I know I am waited long enough) of TTC naturally after the depo shot. I'm 99% sure I'm not ovulating since I've never once gotten a positive test. What type of things should I ask the dr? Are there specific test I need to ask for to see if I'm ovulating or if there is something wrong (PCOS or cyst or something)
