Looks completely normal to me for a breast fed baby. I'm not sure what baby poop looks like if formula fed.

It's he breastfed?  If he is, it's normal

At 4 months old, I'd say that's diarrhea. But don't worry unless he keeps doing it. You might want to try cutting back on the formula and breastfeeding more if possible. See if that helps.
