Birth control you got pregnant on?

I've been on the pill for three years and I want to switch to the patch. But lately I've been reading about people who got pregnant on the patch and I'm wondering if it's less effective than my other options. Thought about the iud but I'm freaked out about the insertion procedure... anyways curious as to which birth control failed the most.
