About to turn 40 and still ttc

I would like some good positive advice but honest too. I have had 3 miscarriages no biological children I'm married and has been for 3yrs I would like to know what help can a gyn doctor give when ttc at my age and this is a new gyn in a new state my previous doctor didn't help just made sure everything was working but never really helped me with trying to convince...
