Labor or not?

My lower back has hurt really bad since last night at 9:30 went away for a little an passed out and now its back. I had painless contractions for a hour last night an now they are back an its 8:20. My stomack hurts an especially right above my vag. I feel like i have to go #2 but cant. My water has not broke. Should i go to the doctors or just wait it out till my appointment in 5 days. I havent dialated according to doctor but extremely thin cervix. 38 weeks an 5 days.
