Looks like eczema.  There are several things to try. With my first I had to use a prescribed hydrocortisone cream, with my 3rd the aveeno baby eczema lotion in a little circle container is working well.  Make sure everything that touches your baby is fragrance free and dye free that may be what is causing it

My baby had something that looks exactly the same. Eczema. Got cortisone 10 from the doctor and cetaphil restoderm. Switched to fragrance free wash and dreft detergent.
