Who's to blame when cheating?

The cheater, absolutely. The person they cheat with -- whether they have knowledge of you or not -- doesn't owe you anything.  Yes, their behavior is ethically questionable, but ultimately they're irrelevant and usually a symptom rather than a cause.

1. The one who is responsible for breaking your trust and cheating is responsible and is the primary blame
2. If the other person they cheated on you with is aware they are also part of the problem 
3. Anyone else who is aware of what's going on and fails to let the person know that someone is abusing their trust then they are also at some kind of fault.

They are all guilty. People cheat when they aren't feeling happy, appreciated, loved, pleased sexually, they aren't making the effort or feel like they aren't a priority, pretty much, their needs aren't being met and look for it elsewhere. The one they cheat with probably knows he's married or has a girlfriend or kids and still continues the relationship. She fill the void that he's been feeling, she's his escape from reality, she makes him happy, she fulfills the need. The one he cheats on is also to blame as well. Relationships are what you make them. If things were good at home he wouldn't feel the need to cheat. Maybe she's neglecting him. Maybe work, kids, family, friends, or an addiction is a priority over him. Maybe they constantly fight. Maybe she sides with her family over him. Who knows. But clearly things will never be the same again. Trust is broken. The relationship is done.

If the person they cheater is cheating with, they should take responsibility for harming someone's feelings. You could say they have no obligation to the one being cheated on, but if they know and can still do it, that doesn't say much about them either.
