Am I fat? I'm 5"2 215 pounds, I'm big because I have lupus & I'm on a couple of medicines that make you gain weight & retain water. it's so hard for me to eat healthy & exercise because I get severe fatigue & I went from being an active high metabolism to having lupus & a slow metabolism, I've been trying to lose weight since I've gotten diagnosed 3 years ago but it's so hard for me, I need help & motivation/encouragement...

You're considered overweight. I know how hard it can be to lose weight even without health issues. But you totally got this! Just try to do the best you can, work out when you can. If you can't avoid junk food yet, at least also eat healthy food so you can get the nutrition you need and also can start making a habit out of it.
