Plz help!!!

So I'm 17 years old. I will be 18 in July. About a week ago, my parents sent me to my grandmothers house. For me being in a relationship with a black man. She had just now figured this out after I had been seeing him since 8th grade and he was going into senior year of high school. I now am 4 hours away from him, and my heart is broken he brought me out of the darkness I was in. Now I have fallen into my old habits of eating disorders, and self harm, and I often just wanna die....I don't know what to do with my self... please don't judge, just help!
