What reasons are there for supervised child visitation?

Just curious, I saw this today at lunch a child visiting his mother with a social services person supervising it all. I was wondering what exactly do you have to do wrong as a mother to warrant supervised child visitation?? Is this a fairly common thing, or do you have to have done something really bad?  I assume, violence, drugs/alcohol or attempting to kidnap them, are a given, but are there other reasons?

Basically any of the reasons you would lose your kid in the first place. 
If you were super super bad... You likely wouldn't get visitation for a while

You pretty much named the most common reasons. A parent needs to be responsible enough to properly care for a child's needs in order to be able to have visitation, and if that parent isn't than a responsible person needs to supervise in order to be sure the child's needs are meet and he/she remains in a safe and loving environment.

It depends on the state and the case.

In some states, something as simple as a custody battle can warrant supervised visitation.
In others, it takes serious offenses such as drug abuse or a history of violence to be put on supervised visitation.

When I was little, my dad had to have supervised visitation with me. The only reason for it was a family feud. My mom's parents had custody of me and they did not trust him to be alone with me because he was in the process of trying to get custody of me and they thought he was going to steal me away from them (which he wasn't). That was the only reason. No drugs or abuse or anything like that. Sometimes supervised visitation is requested by the child's guardians if they don't trust the visiting party. (Just a little background on my situation so it makes more sense: My mom gave me to her parents in a type of non-legally binding adoption at birth without my dad's consent. It took him almost five years to even win visitation rights because my grandfather was just difficult to work with in court.)

Sometimes it's just because the parent doesn't have a good place for the kid to come visit, or if both parents don't get along like if they have restraining orders and what not than they might have supervised visitation so that the kid can be dropped off and picked up without having to see the other parent. It isn't always because of them being a bad parent. My mom had supervised visits

my nephew was only seeing his biological mother with supervised visitation. she was in and out of jail, doing drugs, & prostituting herself. my brother has full custody of him and she's back in jail.
