First Dr Appt - TTC Questions

So today I'm going to the doctor to have my yearly checkup and I'm going to bring up my concerns about TTC. We've been trying for 7 months, unsuccessfully. What are some things I should ask the doctor? Are there any specific tests I should ask them to run?

I'm so nervous, I just know I'll forget something important... thanks for any tips!
