How to transition from co-sleeping to own bed?

My daughter is 7 months old and is currently co-sleeping with my husband (her father) and I. We want to transition her to her own bed. We have a pack n play she can sleep in, however she either sleeps for short periods of time in it or wakes up immediately when I lay her down and starts playing. She is still breastfeeding as of right now. Her pack n play is right next to our bed. How should I go about transitioning her?

My son is 7 months today. I can now proudly say he sleeps in his own bed(pack n'play) through the night!! 🙌🏼 lemme tell ya it took forever!! But I was desperate. 
Let me start off by saying my baby was a nicu baby and slept in his own bed and took a binky by the time we got home. I never planned on bed sharing because it scared me so bad. But with all the clusters feeding(breastfeed) I would fall asleep with him in my arms with a boppy. So he quickly loved to sleep there instead of on his own. Also the binky is now the worse thing on the planet to him😒 didn't last long at all. 
I couldn't sleep at night, I was half awake all the time making sure DH didn't roll over him or smack him in the face. Yeah I could feed him easier but I couldn't even sleep. So we tried a swing for a while n granted it worked till he was about 4m. Sleep regression hit and he grew out of it😬 back to square1. 
So I tried his pack n'play. At this point it was still raised, that way it was the same level as my bed so I had it next to me. And every night I would sleep him in my arms wait about 20ish mins and transition him to the pack n'play, I put him on his boppy tho so he still felt like he was still being held. Little by little every night he would stay in it a little longer. 
Also I used it as a changing station and he would have tummy time in there. I wanted him to feel more comfortable and he grew more familiar with it. 
I made sure every nap while we were home was there in the same spot. He would open his eyes every once and a while to see if I was there so I just made sure to stay in sight and he'd fall back to sleep. If he cried I wouldn't pick him up right away, just talk to him and try to calm him down. 
N when that doesn't work I put my hand just under his little booty not picking him up and bounce him. I swear that is the magic trick it would put him back to sleep so quick and I wouldn't even pick him up. 
Once he could sit up unassisted I finally lowered his bed and I feel like that's when he slept through the night. Now the pack n'play is about 3" from my bed side and he sleeps perfectly. I finally get a nice full night of sleep. 
Every baby is different and you know your baby better than anyone, just be patient and you'll find out what works for you and baby.😊

Right there with you...exactly.  I wish I could get past the initial screaming that happens when I put him in his crib at night. Routines aside...he's pissed that I'm trying to get him to sleep alone. Wish I could get past step 1. 😭😭

Omg the same thing happened with our 7 month old! He slept with us and continued to wake every 2/3 hours and we were super uncomfortable! He needed his own space. I finally had to transition because one morning he fell off the bed. He woke up and was very quiet and just crawled right off...  felt so bad. So I noticed when I first started putting him in the crib he would feel like nothing was around him so he would want to roll around and wake up! I know this is bad and I'm not telling anyone to do it but it worked for us. I would put him to bed between two pillows and it helped him feel secure. After about a week he got in the groove of sleeping in the crib and now we don't use pillows! He rolls and sleeps on his tummy now

Im in the same situation... literally, my boy has never slept more than 2hours 3 tops once he slept 5 hours and i was freaked out! He co-sleeps with us part of the night in between feedings (breastfeed only). The doctor told us that some babies start to sleep more once they are moved to their own room (i haven't tried yet 'cuse he wakes up so much that i just can't be back and fort from his room) but the lactation consultant says that some babies are like that breastfeed co-sleeping or not. I haven't slept more than 2 hours in months 😓 So if you try something let me know i wish i had something better to contribute 🙆🏻
