How can you get pregnant while using condoms? (Even when you check for holes after and there were none and he never ejaculates inside)

Nothing is 100% effective at preventing pregnancy except for abstinence. 
Do what you can and if you are trying to prevent pregnancy be as safe as possible. Use the birth control pill or similar, and use condoms too.

They are only like 97% effective. It says it right on the package. Though I don't actually know anyone that it's happened to.

I do not know 100% why but I have been told because some do have microholes like a balloon does. That's why balloons deflate after awhile because it has microscopic wholes in it that you can't see. Also because possible while the people going at it it can rip then or the condom can be il fitting and sperm can leak out over the top of it. Those are the few reasons I have been told.

Microholes that you won't be able to see. (Yes they do exist). When trying to avoid pregnancy it's best to layer protection (do not put two condoms on. They will break.). I mean a hormonal birth control + condoms, condoms + spermacide, condoms + pull out (don't suggest the pull out method but it's still a method), condoms + vaginal pill, etc. Safe combinations.
