Saying "no" to marriage proposal?!?!?

So this hasn't happened to me but I'm very curious. What happens after a woman says no to being proposed to? 
Does the relationship just end? 
Do they break up right after? 
Is there a serious discussion about why?

I mean I said no to my husband when he asked me the first time. We got pregnant after dating 4 months, and he asked me when I was 7 months pregnant. I told him just because we have a baby doesn't mean we need to be married. And we lived together after 6 months of that I got on his insurance, 2 years later he really proposed and I said yes. We finally knew each other well enough, that we felt 100% comfortable. Our situation wasn't planned we didn't want a baby, but because of it we grew closer and were able to make sure it's what we really wanted. Now we are happily married!!

I guess it depends on the relationship. He could have jumped the gun and proposed too soon... or maybe she doesn't see herself ever marrying him. If it's a "too soon" situation I'm sure he'd keep the ring or she would and maybe propose to him when she was ready. If it's a "woah! No way would I ever see myself marrying you" situation... a break up is order cause they want different things.

Well idk if this applies but in my situation we had a baby soon after we started dating so we've been together two years now and my insurance has been an issue. He refused to marry me because of insurance when I was pregnant so in January I wasn't a student anymore and we were trying to get me insurance. He kept asking everytime insurance got brought up "we just need to get married" "do you want to go to the courthouse" "you need to get on my insurance so we gotta get married" everytime he has said something like that I always respond with "no absolutely not!!!! I am not going to marry you because of insurance. You refused to marry me when I was pregnant so I could have insurance and you made me get on Medicaid to be covered and now is no different. If I have to I'll apply for Medicaid again before I marry you because of that" we are still together and nothing's changed. Marriage is a sore subject for us though so we try to avoid it as much as possible! I know this isn't a full on proposal but it's similar.
