My life just keeps screwing me

So last time i made a post it was because the condom broke. This time, it came off inside of me. Now, this has happened before but opening of the condom was sticking out of me. This time, the entire thing was in me. I just took a plan b 3 days ago on Friday. Do I need to take another one? Is it likely that I could have gotten pregnant from this? Please help!!
