The Condom Fell Off...

I was having sex with my boyfriend, the condom was on and we kept checking to make sure it stayed on.... but once we were done he pulled out and the condom didn't come out with him. It was deep inside me. But it had been on pretty much until the last second because we kept checking to make sure. I think it caught most of the semen but you can never be sure, and this has happened once to me before: last time, I took plan B, even though it wasn't even near my fertile window (so I probably didn't have to take it but I freaked out). This time, <a href="https://play.google.com/store/apps/details?id=com.glow.android.eve">Eve</a> tells me that my fertile window has started, which makes me extra worried. Is it bad to take Plan B more than once? I know I shouldn't rely on it more than emergencies, but I'm not taking any other birth control because I'm afraid of the hormones and long term effects (I'm aware plan b has hormones as well, but obviously I'm not taking it all the time so I'm not worried about the hormones too much). In total, if I take it this time, I'll have only taken plan b TWICE. I know that's not terrible but it's not ideal, either.
Has this ever happened to anyone else? What did you do? How many times can you take plan b?
