medical bill in collections HELP!!! 😣

So i have an almost 4,000 medical bill that is in collections that i cannot afford to pay full up front. it's been in collections over a year but didn't recently find out about until about 9 months ago. I am terrified they are going to access my bank account and drain all my funds my fiance and I have in there. i currently am not working, lost my job about 6 months ago..does anyone know if I call them and set up a payment arrangment to pay monthly payments if they will sue me, or go after my bank account?
