lower back pain labor or bh!?

I'm 38+5d I have been having lower back pain all day some contractions maybe about 3x each hour some travel to front too but mostly my back so I have tried my best tracking but its hard to keep track I usually just go by my pain and when they hit it hurts could this be bh contractions or actually contractions but barley starting this one is diff compared to my first baby who came naturally with him I had no contractions til i had spotted blood ND then pain came hours later nd he did too but i waited to pain was unbearable to go in but this one is a girl and my first it's way diff then my boys I wondering if I should call let Dr know what i feel or just wait to see if they stay nd become closer or unbearable idk I live 40 min away from my Dr need sum advise?
