PCOS or not ?! Help! I have regular periods

Im 30yo, ttc for 8cycles so far for #1. Very regular cycles , I've been getting LH surges every month on opk.
I have persistant acne, ultrasound showed polycystic appearing ovaries around 20each side.
All the hormone levels are normal , except antimûlerian hormone it was a bit high.
Any input ?

Have your day 3 tests and day 21 test done. It will help you figure out which hormones are out of balance. Day 3 tests estrogen, progesterone, LH and FSH and their ratio. PCOS can cause your LH levels to be always high that’s why OPK tests may not be useful as it will always test positive if your LH level is high even if you don’t ovulate. Day 21 is your best detector of ovulation as it measures your progesterone level post ovulation if too low you did not ovulate. Once you figure out which hormones are out of balance you can figure out how to treat them. I suggest starting with easy natural methods before you go invasive. Many women with PCOS can get pregnant naturally and fix their cycles by losing weight, regular exercise, eating PCOS friendly diet, and using Chinese medicine herbs, acupuncture and any form of mediation. I felt the same with my diagnosis after getting the opinion of a second fertility specialist. Trying 2 years. I have regular cycles with positive OPK each month, but have monthly hormonal acne (clinical sign of high androgen despite normal blood test) and polycystic ovaries (about 30 total). Doctor said to be diagnosed you need 2 of 3 of following: 1. Infertility OR irregular cycles, 2. Elevated androgen or clinical signs of elevated androgen such as acne or facial hair, and 3. Polycystic ovaries. My AMH was 5.4 once and 3.84 a few months later which he also said was slightly elevated and could be a sign. Clearly it is a controversial/difficult diagnosis as the first specialist said my high resting follicle count and AMH wasn't PCOS, but just that I was "young" (I'm also 30!)

Did your blood test high for androgens?  I know when I was diagnosed my reproductive endocrinologist stated that they typically diagnose based on three main criteria: irregular period, cystic ovaries, and increased androgens in the blood. My doctor said that if there are two of the three they typically say you have it. When I was diagnosed I had all three. From what your post says seems that you have 2/3
