Silent reflux.... how do you know????

My little guy is 7 weeks old and really struggling in the evenings. He will refuse his bottles and seem to be in a lot of pain. He will get bad breath and spit up clear liquid. Doctor has giving us medication but not sure if to start it if he doesn’t have reflux so confusing. Help needed please new mom so feeling very lost!!!!

My LO has it and as a newborn she got hiccups excessively, her breath would smell sour/acidic, she would act like she was chewing, arching her back while sleeping, and the obvious spitting up. A daily probiotic did wonders for her. We tried to wean off of it when she was a few months and all the symptoms came back. We still give a half a dose of them everyday. I’m too scared to stop them yet and she’s almost 8 months old.
