Painful sex and bleeding?

Hey ladies! Okay so I have a beautiful daughter who was born 12/30/16. My question for you all is this; ever since being given the all clear to have sex again, it’s painful. Like internally painful and I ALWAYS bleed afterward. I had to have an episiotomy during my daughters delivery. It healed good but about a month ago it became very painful and it persisted for about 5 days. I then had my husband look and it and there is a hole (about the size of the end of a ball point pen and fairly deep) in the incision sight. It’s no longer painful but the hole is still there. All of this is really starting to bother me as I have no clue why it’s still painful to have sex and the bleeding that takes place after the act. Has anyone else experienced this?
