diaper rash ?? help

Question

so my daughter has had a few times diaper rashes or the doctor say it yeast infection, but my question is how do I stop it from happening again like I don't know how she is getting it so much like she doesn't sit in her diaper wet or poopy it's changed right away unless she's sleeping.
she's 11months and she's bottle feed with formula and breast milk.
and food is both baby food and some of big people foods.

It happens sometimes, despite our best efforts. Girls are just susceptible to it. 
We’re fighting it too, with my 10 month old. I suggest cutting out dairy and sugar from her food for now, add in a plain probiotic yogurt, and adding baking soda to her bath every day. 
Basically, you have to get the Candida bacteria under control. Keep this up for a week after you notice improvement, and then after that keep plain yogurt in her diet at least 3 times a week (we give it to her with her breakfast), and do the baking soda bath once a week to keep it under control.

Lather a good brand of diaper cream all over her bum and front end(I use sudocrem) , can never use too much, and then put some baby powder on the area as well. That way if she pee’s or poops it’ll hit the powder right away and not her skin with the cream on it
