Has anyone had two different types of birth control fail them?

I got pregnant on Nuva Ring three years ago and not too long ago I got pregnant on the IUD. It had "fell out" and I didn't know. It wasn't in me so I didn't actually get pregnant on it but you get the point. Has anyone has two or more birth control options fail them?
