I am very uncomfortable with my doctor even seeing down there. I don't want her to at all. And I don't want my mom to know I'm sexually active I just don't know what to do

If you're sexually active, you need to tell your doctor. Your doctor has to keep what you tell her private, it's federal law. She may recommend you see a gynecologist for an annual pelvic exam.
