Looks painful and it's probably having a hard time healing because of friction. It sounds weird but I would try bag balm for a week or so and see if it helps. It's thick so it will likely stay on better than anything else and contains antiseptic properties. I use it on any wounds my children or I get and it works wonders.

I’m assuming because it has been there since before you had you child, a doctor has seen this and thinks it’s not an issue ?  It doesn’t look like anything to raise concern to me 🙂
