omg our baby was covered from the neck down in spots similar! took him to the Dr., couldn't really identify what caused it but said as long as there was no fever and it didn't seem to bother him that we shouldn't worry. gave us cortisone cream and Zyrtec just in case it did start bothering him and it went away in a few days on it's own.

Poor baby! My daughter recently had something like this & it was viral. I’d make a trip to see his pediatrician just to be safe!

That's not a normal spot for heat rash. I would contact doctor. That looks more like an allergic reaction to something.
