Hello! So thankful to have this group. I’m facing some difficulty finding a provider to sign a medical form the sperm bank needs for me to register. My Kaiser obgyn says she won’t sign it and is sending me to the “Infertility Reproduction Department” but none of that is covered through insurance and 1 appt is $350! They can sign the form after the initial visit, but the office is also an hour away since it’s specialty and I don’t live in a big city. Who did or is going to do your <a href="https://glowing.com/glow-fertility-program">IUI</a> if you’re going that route? Any OBGYNs more helpful than mine?! Also, who signed your sperm bank medical form if you needed one? Thanks!!
