Help?!

I don’t know what’s going on down there, I have normal discharge and I keep everything clean, no weird smells, but I get really bad itching around the vaginal opening and area around it, sometimes it’s so bad that it bleeds. It’s not constant but when it is i can’t stop thinking about it. I don’t know what to do, I tried changing types of underwear and soaps and laundry detergents and it’s still the same :/
