Protected Sex

So I was reading a few topics and I noticed a lot of people mention that their mate pulls out even while wearing a condom. How much of a difference does this make? I'm curious because my BF and I use condoms but he doesn't pull out when he has one on. I know theirs a risk of it breaking, but when your mate pulls out do y'all get a new condom and continue?? How does that work?
