5m old excessive crying?

My son just turned 5m old on the 4th. (He was born 6 weeks early at 34w6d)  for 2 weeks now around 12-2pm he will begin screaming, like whaling screaming, gagging, holding his breathe, puking screaming. NOTHING absolutely NOTHING! Has worked. Diaper/bottle/tyneol/gas drops/ camilla drops... holding/rocking/shh nothing we have made 3 trips to the pediatrician for this, recording videos for them. They have no answer, other than "give tyneol he has 2 tooth buds, its probably teething" 
I. Can. Not. Keep dealing with this! I literally dread every day, every day I watch the clock knowing hes gonna start it up. Always during his older sisters nap so I end up extra stressed, 2 screaming babies and no supper for my husband when he gets off from work. This is becoming a serious problem.

My babe was waking up screaming and writing in pain in the middle of the night. I had tried Tylenol, gas drops... Nothing helped until I cut dairy out if my diet and we have had complete success: back to sleeping 8-10 hours. Doctor says it's likely a sensitivity since no blood in his diaper and I can try introducing dairy when he's around 6 mo.

There could be several reasons for the extra fussiness. Babies don't necessarily hit growth spurts right on the dot. It could be teething, who knows. Unfortunately it's all a guessing game because our littles can't talk. I know it's frustrating, my son is fussy all day it never ends😕 I started playing more with him, and engaging in new activities. New sounds, different textures(washcloths, blankets, toys, etc) it's helped tremendously! If it's warm where you live, maybe take a short walk to a park, or just around your yard. Sometimes fresh air helps a little too😊 and it's okay to let him cry for a few minutes so that you can take a breather. I call it "mommies time out". If all fails, sometimes I give my LO a warm bath and it seems to help. It won't last forever, you got this Momma!

Our girl is doing the same! Wakes up about an hour into bedtime screaming (not all the time though). She screams so hard she throws up.

My bub started being similar around 3-4pm when he’d normally have his last nap nothing would work til it came dinner times and he’d eat and be good again but would end up overtired cause he never had his last sleep, I started giving him lunch the last few days and it’s helped so much, he’s now been having his last nap easily as well. I don’t know if you’ve started food yet but I have been doing fruit and veggie purée for about a month now and since adding the extra food time he hasn’t screamed and cried for hrs in the afternoons
