Most babies will smile by 2 months of age, you should discuss it with your pediatrician if not smiling by 3 months.

My son will be 7 weeks on Tuesday and the first real smile I got was yesterday and just some slight smiles today but he definitely does not always smile back

My 6 week old is still getting the hang of facial expressions, she only smiles back about 10% of the time. They're still figuring out how facial expressions work etc. Are you in your baby's face smiling at them a lot? Mine mimics more often if I'm really up close talking and smiling at her, and when she smiles I have a big reaction to encourage her to want to do it again.

Her last appointment was last week and they checked her eyes, ears, mouth etc and told me everything seemed normal. Developmental stages can take babies a few weeks, I wouldn't worry yet, she has a few months to get the hang of it!

My daughter doesn't always follow me around either, sometimes I'll be right in her face talking to her and she stares past me or looks in different directions but not at me; it might just be that she's really distracted. But you could ask her pediatrician to check her eyes maybe?
