They gave me the questionnaire at his four month appointment just wondering if they’ll do it again.

I’ve never been screened at any of my daughter’s appointments. I was only screened at my 6 week postpartum visit and never since then.

I can’t remember exactly which appointments I got them for but yeah, definitely a couple of them within the first year.

I absolutely was! I filled out a questionnaire for every single appointment during my pregnancy as well as every one of my daughter's appointments postpartum for the first 12 months.

I got those like 2 or 3 times his first year of appts. The ped ended up saying I was well into PPD territory based on my answers and gave my information to a company that helps women find the right resources to handle PPD.

With my son they did it at 2 months and 4 months. A month later I get a bill from them charging me for the questionnaire. I was livid. I had already done the screening with my insurance nurse as well as my doctor at 6 weeks. So when I had my daughter in for her 2 month wellness I told the receptionist I wasn't doing it. The receptionist was different (and honestly a witch) so I just took it but didn't hand it back. There's no way I'm being charged for them looking over 6 questions, even if it's only $15.
