Thrush and monistat?

When you use monistat for thrush on the nipples do you just break open the capsules? And does that mean you have to buy a few boxes and use one for each application since you're supposed to after every feeding/pumping?

I was told to combine equal parts of monistat, hydrocortisone and polysporin. But wasn't told if it was a fresh mixture each time or if I do a mixture and just keep using it.
