What should I do?

I'm a FTM and when I took my son's nighty off this morning i noticed his toe all bloody. The third picture is after I cleaned it with peroxide and I put aquaphor on it. It bled through his footie pajamas. He never cried and he acts like it doesn't even bother him. 
The first pic is after I cleaned it. There is toenail still there.

You can take him to the doctor or watch it for the next day or two. It looks like it might have been a blood blister from maybe dragging his feet while crawling? My son has a blister on the top of his toe from crawling and dragging he’s feet across the carpet. It never popped but it did go away. Me personally I’d kept tending to it, watch it see if it get any worse, oozes, bleeds, toe nail comes off, check his temp, etc before taking him to the doctor.

What should I do?
