Birth plan template?

My husband and I are going to write down our birth plan tonight...we have the groundwork in mind but want to put it on paper...does anyone have a picture of theirs they don’t mind sharing or have a template they used? I need help covering all the bases and want to make sure I’m not forgetting anything. 
SIDENOTE:: I know you can never fully plan what is going to happen and our main goal and concern is for a safe delivery for both mom and baby! We just want some of our thoughts written down so that it can be written in black and white when we, being first time parents, are undoubtedly going to be off our mental game in the midst of labor!

I think that one super important thing to know is what your hospital already does/encourages if everything is going good! Like mine does delayed cord clamping, skin to skin right away, uninterrupted hour of skin to skin with mom right after delivery. So those things I didn't put on my birth plan

My last one was very simple. I just typed it up on Microsoft and gave a copy to my nurses and the front desk. I just googled different templates and ideas and then made up my own, in bullet point form. So very simple and to the point.

Mine are fairly simple. I don’t care about having music playing or something like that and my OBGYN has simple things that are standard such as skin to skin contact right after birth, delayed cord clamping, and no bath for an hour after birth.
I am going to request tons of ice packs (I was extremely hot with my first and probably went through 10+ of them). I am going to tell them I want an epidural ready but I want to go as long as I can without it.
You should tell them if you want to be able to walk around or do other positions. I will probably also tell them to have Zantac at the ready since last time I forgot mine at home and my hospital had a pharmacy but the doctor had to send in for a prescription of Zantac which then had to be filled and someone had to go grab it. This meant I was in pain from bad heartburn for hours while laboring. It sucked. I will definitely have it packed this time though.
The big ones to write down are how you want to deliver and if things aren’t already standard at your hospital that you want such as delayed cord clamping or if you want an epidural. The rest can be provided easily even if not planned for.
