3wks normal temperature

I was sick for a few days fever of 100.4 he was getting breast milk the whole time yesterday he spit up like 4 times he never spits up today he hasn't let me put him down even to sleep his temp was 98 last night and normal all day has a 99.21 temp now and has been wanting to eat all day. When should I take him to the doctor definitely calling his doctor tomorrow
